Introduce a “health system navigator” for every patient with ongoing or complex needs. This would be a dedicated person (or small team) who helps individuals coordinate their care across GPs, specialists, hospitals, and support services. Instead of patients having to figure everything out themselves—appointments, referrals, follow-ups—they’d have a single point of contact guiding them through the system. This doesn’t just add funding or access—it fixes the fragmentation problem directly, making the whole system easier to use and more efficient at the same time.
